STUDENT ENROLMENT AND CONSENT FORM

Responsible person to Complete

Where a student is under 18 years of age and has a responsible person; their ‘responsible person’ must agree to and sign the Under 18 consent form.

For this purpose, a person is a responsible person in relation to a child if:

  • the person is a parent of the child (except where, because of orders made under the Family Law Act 1975, the person no longer has any parental responsibility for the child); or
  • under a parenting order the child is to live with the person (whether or not the person is a parent of the child); or
  • under a parenting order the person has parental responsibility for the child’s long-term or day-to-day care, welfare and development (whether or not the person is a parent of the child); or
  • the person (whether or not a parent of the child) has guardianship or custody of the child, jointly or otherwise, under an Australian law or a foreign law, whether because of adoption, operation of law, an order of a court or otherwise.

Orientation and Attendance 

Orientation is held at the start of Frenchie for a Day and outlines The French Beauty Academy’s expectations of student participation. It is expected that you, as the responsible person, discuss the training program as outlined with your child to ensure participation requirements are understood. Clarification should be sought where necessary.

Classes are scheduled differently from secondary education classes and students are expected to manage their own time for themselves in a way that optimises their learning experience.

If a class finishes prior to the scheduled time, students will be released from class and will not be supervised.

 

Classroom and Learning 

The French Beauty Academy is an adult learning environment, and all students are expected to participate in a mature manner.

Adults use differing motivations, interests, attitudes, values and experience when they learn and teaching methods will be different from those encountered in a secondary school environment. All students will be required to actively participate in group activities and use their own judgement in decision making.

Students are responsible for their own learning and are expected to manage their activities, seeking assistance from educators when needed.

Students under 18 years of age share facilities such at The French Beauty Academy campus with adult students and adult workers.

 

Health & Safety 

The French Beauty Academy provides an environment that supports the health and wellbeing of all students. The French Beauty Academy has obligations under the Work Health & Safety Act 2011 that includes but is not limited to;

  • providing and maintaining a safe and healthy environment;
  • providing information, instruction, training and supervision to ensure health and safety.

 

Students have obligations under the Act that includes but is not limited to;

  • complying with the instructions given for work health and safety;
  • not to willfully or recklessly interfere with or misuse anything provided for work health and safety;
  • not to willfully place at risk the work health and safety of any person;
  • not to willfully injure himself or herself.
