I understand that I am undertaking a course that is subsidised by the QLD Government Certificate 3 Guarantee program and am aware of the Student Fact Sheet available on the Departments training website
I understand that I am only eligible to undertake one funded/subsidised Certificate 3 qualification (with the only exceptions being if completed a course as part of VETiS OR you are required to complete a foundations skills course prior to completing the Certificate III)
I have read and discussed the course information with the Student Advisors to ensure that this course meets my requirements. I am aware of the course entry requirements, duration, outline, outcomes and other relevant information.
I have access to The French Beauty Academy Student Handbook and am aware of the support services available if required and have read and understand The French Beauty Academy’s Refund Policy.

I understand that I must complete a post-course survey within 3 months of completing or withdrawing from this course.
Applications are subject to approval by The French Beauty Academy. Submitting a completed application does not guarantee approval of funding. Training cannot commence until you have received written confirmation of your application being approved.
Availability of funding is subject to change without notice at the discretion of the Queensland Government.
Applicants are only eligible to receive funding for one (1) Certificate III Qualification.
Students must provide additional information / feedback to The French Beauty Academy within three months of completing their qualification.
Student co-contribution fees must be paid to The French Beauty Academy in full prior to the course commencement or through a payment plan over the duration of the course.
Additional terms and conditions will be outlined on the application forms and are subject to change.
Students may be required to purchase items to supplement course inclusions, these costs are not included in the student contribution fee or funding subsidy.


PRIVACY STATEMENT
The Queensland Government allocates funding for students to undertake qualifications under the Certificate 3 Guarantee Program. The French Beauty Academy has been approved to deliver training and assessment for qualifications which eligible students will be entitled to undertake under the Certificate 3 Guarantee Program. The personal information you provide will be collected and used by The French Beauty Academy for the purposes of:
assessing your eligibility for the Certificate 3 Guarantee Program;
if you are eligible to participate in the Certificate 3 Guarantee Program, all aspects of enrolment, administration and delivery of the qualification; and advising your Job Active Provider and/or Employer (if applicable) of your participation and attendance in training.

The French Beauty Academy may also collect and disclose your personal information to the Queensland Government Department of Education and Training (DESBT) and other Australian Government agencies for the purposes of:
confirming your eligibility for participation in this program;
informing DESBT that you have enrolled in an approved qualification;
informing DESBT of your completion, non-completion or withdrawal from an approved qualification;
reporting to DESBT’s Ministers and other Members of Parliament on the Certificate 3 Guarantee Program;
monitoring the service given by The French Beauty Academy to you and your satisfaction with the Certificate 3 Guarantee Program; and
DESBT generally administering the Certificate 3 Guarantee Program.

The French Beauty Academy, may also disclose your personal information to another person, body or agency without your consent where authorised or required by law.
